To confirm this new rate series, run the following test quote:
$1,000/mo 20C&L 35 female, beginning one month after the purchase date = $291,580.00
Make sure to use a 2012 quote, funding, and Benefit start date.

Premium or an executed lock-in form under rate series PLA189 must be submitted to the Home Office no later than Friday, December 30, 2011.
